In today's environment, the Self-Learning Module (SLM) is a preferred learning resource that is used in all public schools, with modular distance learning as an alternate learning modality to continue teaching. It is designed to suit the learners' learning needs by providing inputs, tasks, activities, and personalized instruction that allow students to discover and absorb concepts before applying them in real-life situations with the support of their parents as learning facilitators at home. In general, the objective is to create a school- based paperless modular application that will provide a more accessible monitoring system with functional sustainability, usability, reliability, security, and performance efficiency. To address the objectives of the study, the researchers used a mixed method research design and the Iterative Model. The study respondents were the principal, teachers, parents and the school administrators of the Santiago Integrated School-Iriga. The data was obtained using surveys, an interview, observation, and the data was evaluated and interpreted using a weighted mean and paired sample t-test. The research findings indicate that there are problems with recording the module monitoring papers, storing module monitoring papers, and retrieving module monitoring papers in the school record module monitoring. In addition, there is a significant difference in user satisfaction between using the online module monitoring and paper monitoring. Thus, the overall interpretation revealed that the suggested system is moderately acceptable to the respondents. As a result, this is an excellent time to implement the Santiago Integrated School - Iriga: Online Module Monitoring Web Application.
